4-page printable self-reported daily log that helps to monitor food, drinks, bowel habits, mood and physical activity that can be used in any specialty of nutrition or other medical fields.

The first page are the instructions and description on how to use the diary.

The 2nd page is the log of sleep, menstruation, affirmations, gratitude and daily planning or goal setting, as well as physical activity and stress.  The table includes spaces for entering food, amount of food, time of eating, medication and supplement use, mood at the time of eating, any symptoms throughout the day, and bowel habits. You can also log in your drinks including alcohol.

3r page is more of a convenience. Food, mood and poop diary is a in A4 paper format. If you want a smaller version, page 3 includes 2 diaries

4th page is a Bristol Stool Chart (Adopted from Rome Foundation), which is useful in identifying the stool type
