I developed this resource from my experience working with chronically-ill older adults.  My patients were often able to verbalize their long-term goals but struggled to assemble an achievable plan and maintain motivation along the way.

  • Notes page: record successes and slip-ups, what worked and what didn’t, observations made, and any other details of their journey toward success
