Medical Information Organizer

Want to help your patients be (much) less stressed when it comes to their medical care?

Whether someone needs emergency medical services or is concerned about an upcoming appointment, assembling and organizing their medical information is the last thing that they should have to worry about.

I created this resource after working as a Home Health RD and seeing countless older adult patients struggle to keep track of this essential information.  In the most extreme case, I was frantically scribbling down a patient’s medication list as the ambulance approached their house!  Though it was designed with the geriatric population in mind, this resource is perfect for patients of any age and with any range of diagnoses.

Use this template to record:

• Patient identification information
• Medical history & medication list
• Care team contacts
• Insurance
• Medical POA & emergency contacts

Pro Tip: Encourage your patients to secure this form and other pertinent paperwork (i.e. insurance cards, DNR paperwork, etc.) in a folder and store it in a safe yet easily-accessible location.  It is also ideal to share a copy with a trusted family member or friend and their medical Power Of Attorney.
